Once your blood test results are ready, we record a concise video specifically for you. This personalized video is delivered alongside your test report at no extra cost, enhancing your understanding and ability to act on the information provided.

Yes, our community is open to anyone interested in health and fitness. Blood testing is recommended but not required for participation.
5. What should I expect after I receive my blood test results?
You will receive a detailed report explaining your results and any notable findings. Based on these, we may recommend lifestyle changes, further testing, or consultation with our experts.
6. How often should I take a blood test with Form?
The frequency of testing depends on your individual lifestyle goals and conditions. Generally, we recommend retesting every 6 to 12 months to monitor changes and adjust your lifestyle plan.
7. Is my information kept confidential?
Absolutely. Form adheres to strict data privacy and security measures to ensure that your personal and health information is always protected.
8. How does the blood collection process work?
After you make a purchase, you'll receive an email with a link to our Calendly to schedule your appointment. Please choose a day and time that suits you, keeping in mind that we require bookings to be made at least 24 hours in advance to arrange the collection. We ask that you allow a 2-hour window on the scheduled day to ensure you are available when the nurse arrives, though the actual collection process usually takes much less time.
9. When should I have the blood draw?
To ensure the most accurate results, especially for tests sensitive to time of day such as testosterone levels, we recommend scheduling blood draws in the morning. Our available times are from 8 AM to 1 PM, as tests are best done while fasted.
10. How will the nurse know where to find me or contact me?
When you book your appointment through Calendly, you will provide your full address, including a Google Maps URL, and your WhatsApp number for direct communication. This information ensures that the nurse can easily find and contact you if necessary.
11. Are the nurses well-trained?
Yes, our nurses are highly trained professionals, provided by Bali's leading pharmacy laboratory team. They are skilled in blood collection techniques and ensure a safe and hygienic process.
