Refund Eligibility You can cancel your appointment and request a full refund if you do so at least 24 hours before your scheduled blood collection time. This allows us adequate time to halt the process and prevent logistical issues. To cancel, please contact us directly via WhatsApp or cancel@formbali.com.

Cancellation Procedure To initiate a cancellation, please reach out to us via WhatsApp or cancel@formbali.com. Please provide your name, appointment details, and reason for cancellation. We will process your cancellation and refund promptly.

No-Show and Late Cancellation If you cancel less than 24 hours before your appointment or do not show up, unfortunately, we cannot offer a refund. This policy is in place because resources and staffing have been allocated to your appointment.
